چکیده

Objective: To describe a new kind of 360° fixation in pyogenic spontaneous spondylodiscitis of L4L5 after anterior debridement and fusion. Summary of Background Data: The adequate surgical treatment modalities for pyogenic spontaneous spondylodiscitis has not been unequivocally defined until present, especially in the use of metal instruments at such a septic focus. Methods: The 54-year patient, who was in an active period of pyogenic spondylitis, treated with surgical intervention involved aggressive anterior debridement and removal of the infected vertebral body of L4 -L5, followed by reconstruction of the anterior column using autologous iliac bone strut and anterior titanium screw-rod fixation (in L4 and L5 residual vertebral body), and posterior pedicle screw fixation in L3 and S1. The patient was given antibiotic treatment with cefotiam, initially with intravenous administration, which was switched to oral administration after 21 days. Results: No further relapse of spondylodiscitis have been detected during routine clinical and radiographic check-ups, with ongoing follow-up. The inflammatory parameters have been in the normal range. The patient is well mobilized, using walking feet for long distances, and she has no obvious pain. The radiographs showed the disappearance of extramedullary abscess, no changes in the implant such as loosening of the screws or fracture, iliac bone graft absorption collapse, and partial fusion between vertebral body and bone graft. Conclusion: This new 360° fixation may be a good surgical treatment alternative, especially in an active period of pyogenic spondylitis, with significant and irregular bone defects.

Posterior Titanium Screw Fixation without Debridement of Infected Tissue for the Treatment of Thoracolumbar Spontaneous Pyogenic Spondylodiscitis

STUDY DESIGN Retrospective study. PURPOSE The aim of our study was to analyze the safety and effectiveness of posterior pedicle screw fixation for treatment of pyogenic spondylodiscitis (PSD) without formal debridement of the infected tissue. Comparison of two-stage open versus percutaneous pedicle screw fixation in treating pyogenic spondylodiscitis

BACKGROUND Percutaneous pedicle screw instrumentation is a minimally invasive surgical technique; however, the effects of using percutaneous pedicle screw fixation in treating patients with spinal infections have not yet been well demonstrated. The aim of this study, therefore, was to determine whether percutaneous posterior pedicle screw instrumentation is superior to the traditional open appr...

Rigid Posterior Lumbopelvic Fixation without Formal Debridement for Pyogenic Vertebral Diskitis and Osteomyelitis Involving the Lumbosacral Junction: Technical Report

BACKGROUND Pelvic fixation with S2-alar-iliac (S2AI) screws can increase the rigidity of a lumbosacral construct, which may promote bone healing, improve antibiotic delivery to infected tissues, and avoid L5-S1 pseudarthrosis. PURPOSE To describe the use of single-stage posterior fixation without debridement for the treatment of pyogenic vertebral diskitis and osteomyelitis (PVDO) at the lumb...

Management of Postoperative Spondylodiscitis with and without Internal Fixation.

Postoperative spondylodiscitis is relatively uncommon. This complication is associated with increased cost, and long-term of inability to work, and even morbidity. Although the majority of postoperative spondylodiscitis cases can be well managed by conservative treatment, postoperative spondylodiscitis after internal fixation and those cases that are unresponsive to the conservative treatment p...

Treatment of Spinal Tuberculosis by Debridement, Interbody Fusion and Internal Fixation via Posterior Approach Only

Surgical treatment for spinal tuberculosis includes focal tuberculosis debridement, segmental stability reconstruction, neural decompression and kyphotic deformity correction. For the lesions mainly involved anterior and middle column of the spine, anterior operation of debridement and fusion with internal fixation has been becoming the most frequently used surgical technique for the spinal tub...
